SUCCESS
SUCCESS is a school-based program serving students from kindergarten through age 21 and their families. SUCCESS Case Managers provide intensive case management services to students and families to address social emotional wellness, basic needs, education, health, and community supports.
Case managers work closely with the students and families they serve to address identified goals and to support the parent/guardian in advocating for themselves and their family.
Students are identified for the SUCCESS Program based on chronic absenteeism, behavior referrals, or being disengaged from school.
The SUCCESS Program manages the outreach and re-engagement process for Des Moines Public Schools.
SUCCESS is currently located in 29 elementary schools, 10 middle schools, five high schools, one alternative high school, and one high school program.
