Obtaining Data on Students Served
One of the benefits of being an official community partner with Des Moines Public Schools is having access to certain data on the students served. Aggregate data is available without a signed parent release for groups larger than 10. If individual student data is requested, a parent release must be on file. Please find the file link below.
In order to obtain data, please follow these steps below.
Step 1: Work with your school’s Community School Coordinator to distribute the VIRTUAL parent or guardian release form to all participating students’ parents or guardians. NOTE: When specific student or personally identifiable student data is requested, a signed release from a parent or guardian is required per The Family Educational Rights and Privacy Act (FERPA). Only VIRTUAL release forms will be accepted.
Step 2: Collaborate with your building CSC to tag students to your program
- This ensures that the data team can swiftly collect the proper information for your program. We are unable to look students up line by line in an efficient manner to serve all requests.
- Students can also be added using this form: Add/Drop Form. Please note that this form only updates with Infinite Campus once per week.
- You may contact the building CSC or 21st Century Coordinator directly to have them tag the students into Infinite Campus. These updates should reflect immediately upon the CSC’s completion of the entries.
Step 3: Complete a Data Request Form
Step 4: Des Moines Public Schools Data, Assessment, and Evaluation Team will be in touch within 2 weeks with program data.
Tagging Students into your Program
Every Monday, Community Partners will receive communication via email from your school’s Community School Coordinator with the list of students participating in the program. Partners are to use the Add/Drop Form for any students that are no longer in their program or are added to their program.

Data Request Requirements
Commonly requested data elements: student attendance, behavior, grades, assessments, improvement from prior year
With Parental Consent:
- Individual, identified student information
- Ex. John Smith: requested fields
Without Parental Consent:
- Aggregate data of student information for groups larger than 10. Groups smaller than 10 are not eligible for any information without a release.
- Ex. Count or percentage: chronically absent students, students with at least 1 behavior event, students meeting benchmark on assessment, students failing or not failing any courses
- Individual student information (de-identified and anonymous)
- Ex. Student 1,2,3, etc.: requested fields as available*
*Information (school, gender, grade, race/ethnicity) may be redacted to maintain student privacy
